Requesting Incident Reports |

Residents desiring incident reports may obtain copies through the Fire Administration Office. Requests for fire reports must be in writing and must be accompanied by a check in the amount of $20.00 made payable to the City of Glendale. Without exception, the written request must include the incident number or the exact date and address of the incident. All requests are subject to review and approval by the Fire Department, and require about one week for processing.
Requests for medical incident reports are subject to the same requirements as fire reports, noted above. In addition, these reports will only be released to the individual who received treatment, or that person's legal representative in possession of properly executed medical release forms. |

Reporting Hazardous Conditions |

Residents reporting hazardous conditions should choose one of the following procedures:
- For life-threatening emergencies, call 9-1-1 without delay.
- For environmental issues and hazards that require immediate response (for example, someone dumping a chemical into a gutter), call 9-1-1 without delay.
- For cases that are not life-threatening, and are related to Vegetation Management (for example, an excess of brush on an adjacent property), call the Fire Prevention Bureau at (818) 548-3814 and report your complaint. The department will get your message and the appropriate personnel or fire company will be directed to investigate.
- For cases that are not life-threatening, and are NOT related to Vegetation Management, call the Fire Prevention Bureau at (818) 548-4810 and ask to report a hazardous condition. A staff member will take down the pertinent information and the appropriate personnel or fire company will investigate.
- If you have any doubt about the appropriate action, call 9-1-1.
